1. Determine what courses you plan to register for at CSCC. Consult with your home institution academic advisor on course selection and equivalency. You may also use the Transferology system.      **Please note: Some course numbers have changed as of Autumn 2016. These changes may not be reflected on Transferology as of this time.**

  2. Upload a copy of your unofficial transcript, Advising Report, degree audit report, or grade report in the indicated area below.

3. If you have earned AP or IB credit please submit a copy of your score report. We are unable to use AP/IB credit posted to another school's transcript.

4. Read Acknowledgment Statement below and check box to submit form to the Center for Advising Support and Exploration (CASE).


List the Columbus State course department and course number of any courses you wish to register for. Be sure to put commas (,) in-between each course you are requesting. Ex: HIST 1151, ENGL 1100, PHYS 1201. Please note this information must reflect the Columbus State course offerings.      
Proof of Pre-requisite Completion
A member of our CASE team will review your previous coursework to verify that you meet the pre-requisites to take the course you have listed above. In some cases, Advisors may not be able to verify course equivalents (ex: college not listed on equivalency database, college is out of state, etc). If you have attended multiple colleges, please upload documentation from all colleges attended.

Before submitting this request, please carefully read and acknowledge the following:
  After a member of CASE reviews your completed course work for prerequisites you will receive e-mail notification confirming or denying your capability to self-register courses within 7-10 business days.

Upon completion of your course work at CSCC, you will need to request a copy of your official transcript from Columbus State be sent back to your home institution.
  Unofficial documentation attached to this email will not be retained by CSCC. If you wish to register for future semesters please submit official documentation to the College. Any changes to registration can be completed via the Cougarweb portal as long as space is available in the course.
 

 I understand that, upon completion of this registration, I may incur Columbus State's $50 Application, Records, and ID Fee.

The fee is assessed at the time of initial registration, and is non-refundable. For students who were not charged and/or did not pay a Matriculation fee or Application, Records, and ID fee during a prior quarter/semester for any reason, the Application, Records, and ID fee is charged and due the current semester of registration in which the fee is invoiced to the student’s account.
